You may fill out your pledge card online by using the link below and then opening the form in Adobe Reader or Adobe Acrobat; when finished, click the submit button at the top right of the form and your information will be securely sent to the Church's Financial Secretary.

Form Instructions:
- Click the link below and choose "Download" to open the form in Adobe Reader.
- Fill in the fields outlined in red (note that the field descriptions are below the fields)
- Optionally, in the signature block you may digitally sign the form; you may use an already created digital signature or you will have the option of creating one.
- Click the Submit the Form button at the top right of the form to send your data.
